If you think about the mechanics of your neck, you’ll realize it’s not surprising how easily it can be injured. It has amazing flexibility, is constantly on the move, has very little muscular support, but has to support the 14 – 16 pounds of your head.

The neck is like the superhighway for your body in that nerve impulses are sent to every part of the body through the nerves that run through your neck reaching out to all areas of your body. From your cervical vertebrae, impulses are sent to your arms, and some into your upper back. Thus pain in your arm, such as numbness, tingling, cold, aching, or “pins and needles”, may be the result of an injury or issue with neck. People with neck problems may mistake their pain for carpal tunnel syndrome, a condition in the hands that is often caused by repeating the same repetitive motions for long periods.

Neck problems can also lead to headaches, muscle spasms in the shoulders and upper back, ringing in the ears (tinnitus), inflammation in the middle ear (otitis media), temporomandibular joint dysfunction (TMJ), or simply a restricted range of motion and severe tightness in the neck and upper back.

Causes of neck and upper back pain

Most times, neck and upper back pain is the result of not just one factor. Injury may be combined with poor posture, subluxations (misaligned spinal vertebrae), mental stress, or disc problems.

Injuries – Easily the most common injury to the neck is whiplash, the sudden, violent whipping motion of the head in an accident. It is most often associated with a car accidents, but it can happen when playing sport, or any other time the head suffers this particular whipping trauma. When this happens, the supporting muscles, ligaments and other connective tissues in the neck and upper back are damaged, but the full effects of this may not be felt for weeks or even months. This is why anyone who suspects they may have suffered whiplash should be extremely careful not to aggravate it, even if they feel no immediate ill-effects. An assessment should be sought from a chiropractor, in case treatment is needed to minimize the effects and guard against possibly permanent damage. 50% of whiplash victims in one study were shown to still be suffering the effects of the injury many years after settling their insurance claims.

Poor Posture – Poor posture is one of the most common causes of neck pain, and can also cause headaches. Unlike sudden injuries, which we usually know have happened, bad posture occurs over time and is something we soon begin to feel as normal. Everyday activities such as reading in bed can create issues and pain later on, and anyone working on a computer for extended periods is certainly susceptible to developing a forward head posture (FHP) and spinal subluxations. The rule is to keep your neck in a neutral position if possible. Avoid craning or hunching the neck, or remaining in the same position for too long. If this is unavoidable, try to practice correct posture to minimize any problems. Ideally, the head should be kept in a neutral position, back supported, knees slightly lower than your hips, and arms supported.

Subluxations – These are extremely common in the neck and upper back area, thanks to the weight of your head resting on such an unstable part of your spine. The majority of subluxations are centered around four areas:

  • The top of the cervical spine where it meets the skull
  • The middle of the cervical spine which supports the greatest weight of the head.
  • The transition between the cervical and thoracic areas of the spine.
  • The middle of the thoracic spine which supports the greatest weight of the upper body

Stress – Mental and emotional stress causes the muscles to tense, especially those in the back and neck. This ‘muscle guarding’ is a primitive survival response to avoid injury in the face of danger, but it ends up causing injury when it happens for any length of time. If pinching the trapezius muscle causes you to wince, that is a sure sign your muscles are tight and trigger points almost certainly exist. Exercise and deep breathing/relaxation techniques are useful for combating stress.

Herniated discs – A herniated or bulging disc will put pressure on the nerves that exit the spine through that area. Lumbar discs are more prone to herniating than cervical discs, but trauma such as whiplash can provoke a herniated cervical disc.

A consultation with your doctor of chiropractic should be scheduled if you are experiencing any pain in your neck or upper back that does not resolve very quickly.

Ideally, the head should be kept in a neutral position, back supported, knees slightly lower than your hips, and arms supported.

Subluxations – These are extremely common in the neck and upper back area, thanks to the weight of your head resting on such an unstable part of your spine. The majority of subluxations are centered around four areas:

  • The top of the cervical spine where it meets the skull
  • The middle of the cervical spine which supports the greatest weight of the head.
  • The transition between the cervical and thoracic areas of the spine.
  • The middle of the thoracic spine which supports the greatest weight of the upper body

Stress – Mental and emotional stress causes the muscles to tense, especially those in the back and neck. This ‘muscle guarding’ is a primitive survival response to avoid injury in the face of danger, but it ends up causing injury when it happens for any length of time. If pinching the trapezius muscle causes you to wince, that is a sure sign your muscles are tight and trigger points almost certainly exist. Exercise and deep breathing/relaxation techniques are useful for combating stress.

Herniated discs – A herniated or bulging disc will put pressure on the nerves that exit the spine through that area. Lumbar discs are more prone to herniating than cervical discs, but trauma such as whiplash can provoke a herniated cervical disc.

What is fibromyalgia?

The excessive amount of ground substance in a fibromyalgia sufferer means that the injured area does not heal properly, leading to the muscle knots characteristic of the condition.

While lupus can be definitively diagnosed with laboratory tests, there are no such surefire tests for fibromyalgia. However, there are a number of tests that can help to rule out certain other disorders, and a physical examination will identify if a person has the characteristic tender areas on the back of the neck, shoulders, sternum, lower back, hips, shins, elbows, or knees. Unfortunately, this lack of 100% certainty can lead some doctors to dismiss a patient’s suffering as “all in the mind”, or – not very much better – to conclude that there is no effective remedy on offer.

This is not true. While fibromyalgia is certainly not easy to treat, a combination of chiropractic, trigger point therapy, massage, and lifestyle changes (diet and exercise) can prove effective in reducing the severity of the condition.

Myofascitis is the result of inflammation caused by overuse or injury to the muscles, is usually related to a specific activity or injury, and manifests quite suddenly. Fibromyalgia is the result of stress-induced changes to the metabolism and healing process, and appears in a slow, creeping fashion, most often starting in early adulthood. Correctly differentiating between the two is crucial as the relative treatments are nothing like the same.

What is fibromyalgia?

Fibromyalgia is a chronic condition that may never go away your entire life, although on the plus side, it does not cause any damage to your joints, muscles, or internal organs.

The most up-to-date research suggests that fibromyalgia is a stress-induced condition related to Systemic Lupus Erythematosis (usually just referred to as ‘lupus’) and Chronic Fatigue Syndrome. All of these conditions target women far more than men, and are marked by similar symptoms such as chronic fatigue, disturbed sleep, and IBS. The main difference is with lupus, which is an autoimmune reaction that attacks the healthy tissues of the body. With fibromyalgia, metabolic abnormalities are the primary issue, causing decreased blood flow to the pituitary gland in the brain. This then lowers certain important hormones, such as those responsible for releasing growth hormone and stimulating the thyroid. Muscle healing is adversely affected, memory and cognition are damaged, and full-blown hypothyroidism may even result.

Specifically, fibromyalgia causes a buildup in the muscle of a protein called “ground substance”. This is found in muscle, bone and connective tissues everywhere in the body, and serves to make these structures stronger and less prone to injury. The excessive amount of ground substance in a fibromyalgia sufferer means that the injured area does not heal properly, leading to the muscle knots characteristic of the condition.

While lupus can be definitively diagnosed with laboratory tests, there are no such surefire tests for fibromyalgia. However, there are a number of tests that can help to rule out certain other disorders, and a physical examination will identify if a person has the characteristic tender areas on the back of the neck, shoulders, sternum, lower back, hips, shins, elbows, or knees. Unfortunately, this lack of 100% certainty can lead some doctors to dismiss a patient’s suffering as “all in the mind”, or – not very much better – to conclude that there is no effective remedy on offer.

This is not true. While fibromyalgia is certainly not easy to treat, a combination of chiropractic, trigger point therapy, massage, and lifestyle changes (diet and exercise) can prove effective in reducing the severity of the condition.

Chiropractic treatment of fibromyalgia

Fibromyalgia causes muscles to tighten and lose their natural pliability, which triggers the spine to decrease its own flexibility, which leads the muscles to tighten even more. It is a vicious cycle that can get worse and worse, causing ever greater pain. Chiropractic care is therefore essential as it ensures that sufferers do not lose too much movement from their spine and muscles. The spine must be properly adjusted and kept moving to counter the insidious creep of fibromyalgia. Treatments should happen three to four times per month, and these sessions will be gentler than normal to take into account the muscles’ susceptibility to injury and their lack of healing ability.

PERIPHERAL NEUROPATHY/ PERIPHERAL NEURITIS

Do you experience burning, tingling, numbness, or sharp nerve pain? These unwelcomed sensations are commonly affiliated with a condition called Peripheral Neuropathy, which concern disorders of the peripheral nerves. Damage to the peripheral nerves accounts for the burdensome sensations, and it is most present in the hands and feet. Other areas of the body can be affected as well.  It is estimated that more than 20 million people in the United States suffer from this debilitating condition. However, conventional medicine, namely prescriptions and injections, do not provide effective improvement or relief, but rather only cover up the symptoms temporarily.

What causes Peripheral Neuropathy?

The Peripheral Nervous System (PNS) relays messages from the CNS (brain and spinal cord) to the rest of the body. The PNS can be compromised by traumatic injuries, infections, genetic predispositions, environmental exposures, autoimmune conditions, and metabolic disorders. Vitamin deficiencies have been linked to be a common cause for peripheral neuropathy as well.  Diabetics, however, are 50 percent more likely to develop and suffer from Peripheral Neuropathy, and those taking cholesterol lowering medication is also subject to this disorder.

Signs and Symptoms of Peripheral Neuropathy

Weakness, numbness and tingling in the hands and feet are the earlier signs of nerve damage. The onset of such sensations may be gradual and may proceed to spread to the rest of the arms and legs. More unpleasant symptoms include sharp, stabbing, and burning pain. When motor nerves are affected, muscle weakness and paralysis can set in as well. The combination of damaged sensory and motor nerves creates a dangerous situation where the patient will likely experience lack of coordination and falling.

Because the PNS communicates with the rest of the body, other areas of the body can present with symptoms as well. Severe symptoms can include atrophy and organ and gland dysfunction. When nerves affecting internal organs are damaged, it can result in problems with sexual function, breathing, and urination. Extreme cases can result in organ failure as well.

Recognizing and diagnosing Peripheral Neuropathy early is essential for effective management, but it is even more so vital for preventing further nerve damage. When should you see a doctor? As soon as you notice unfamiliar symptoms and sensations in your hands and feet, and especially if you suffer from diabetes or take medication on a regular basis.

Studies show that one of the leading cause of death in senior citizens are falling incidents. Correspondingly, seniors often suffer from peripheral neuropathy, and the lack of sensation or the over-stimulation of pain, depending on what type of nerve is affected, in their extremities prove detrimental to their balance and coordination.

Treatment for Peripheral Neuropathy

Your first step is to identify the contributing factors to your peripheral nerve damage. In order to do so, your physician will ask you to provide extensive information into your medical and lifestyle history, including any infections, toxic exposure, medications, autoimmune disorders. If you’ve had any lab work done, such as blood, urine, and stool, you will be asked to bring those as well. Additional labs may be ordered to further assess your condition. Preventing further nerve damage is just as critical as recovering the ones that have already been affected. In some cases, correcting the underlying cause(s) alone can be sufficient for neuropathy to resolve on its own.
