GENERAL HEALTH INSURANCE
Sugarloaf Pain & Rehab accepts most health insurances. However, it is important to keep in mind that no insurance is the same, even within the same carrier. The patient’s individual benefits are specific to that patient’s contracted plan. As a result, we cannot provide a general fee but are happy to take the time to investigate what your specific plan benefits can utilized at our facility and what costs to expect.

- United Health Care (UHC)

***It is important to note that not all services and therapies performed at Sugarloaf Pain & Rehab are covered by insurance and may incur an additional fee, which will be communicated prior to services being rendered.
AUTO INJURY PAYMENTS
Patients undergoing treatment for injuries obtained from an auto accident are not required to pay for their appointments the day services are rendered. Auto accident patients’ medical bills are processed through an injury claim that has been filed with the responsible car insurance. It’s important to note that these insurance companies are different from regular health insurance companies. The bills that have accrued over the course of their injury care at our facility is sent to either the insurance claim adjuster or the attorney on record for the patient. It is recommended that auto injury patients hire an attorney as the attorney bears the burden of the medical bills, as opposed to the patients themselves. Furthermore, PI attorneys are capable of conducting any necessary negotiation should insurance payment be insufficient for all bills accrued.
SELF-PAY / UNINSURED
In the absence of health insurance, Sugarloaf Pain & Rehab offers a self-pay option for treatments. The payment will depend on a variety of factors related to your condition, but we will generally be able to provide you with an up-front rate upon inquiry.
